Complaint form
NOTE: One can file a complaint only for incidents that occur during the time the audiologist or speech-language pathologist who is the subject of the complaint is a registered member of the College.
All complaints must be submitted electronically.
Please follow these steps to submit the complaint form:
- Download the complaint form or open it in your browser.
- Fill out the form, then print it out.
- Sign the printed form.
- Scan the signed document and save it as a PDF, or take a photo of each form page.
- Go to our Google Complaint Form and upload the PDF there. This ensures that the complaint is submitted directly to the Registrar in a secure and confidential manner.
